Please note:

  • VPN remote access is granted based on affiliation with the University. This form is to request access to Yale VPN services for systems or ineligible individuals.
  • Lead Administrators\Operations Managers must submit this form on behalf of the applicant and sponsor. After you complete this online form, it will be emailed to the Lead Administrator\Operations Manager for approval. See Procedure 1601 PR.04 and the VPN service page for more information on eligibility
