VPN Access Request

Please note:

  • Lead Administrators must submit this form on behalf of the applicant and sponsor. After you complete this online form, it will be emailed to the Lead Administrator for approval.
  • This form is to request access to Yale VPN services for systems or ineligible individuals.
  • See Procedure 1601 PR.04 and the VPN service page for more information on eligibility.
  • Library systems are independently managed. Visit the Yale University Library website to learn more.
